Plan Provisions
The following are applicable to both above plans:
- Payment is due at the time of service.
- Benefits last for one year from the date of enrollment.
- Unused benefits within the purchase year are forfeited.
- The plan is non-refundable, non-transferable, and cannot be combined with other offers or insurance.
- For full discount eligibility on treatment, appointments must be scheduled as recommended.
- Only a 5% discount applies if plan use is discontinued midyear.
Note:Package deals are exempt from discounts. Neither are insurance plans, rather service arrangements.
